Remote Monitoring Plans

Our remote monitoring services help you to stay on track as you follow our treatment plan, thus improving control over your health over a sustained period.

All you need to do is:

  • Enter your readings in your trackers as instructed.
  • The values will be reviewed by your doctor and you will receive feedback online.

Basic Diabetes
This program is meant for patients with diabetes on Oral Anti Diabetic (OAD) medicines. In this program, the patient can report blood sugar, insulin doses, dietary changes, blood pressure, lipid profile and weight readings.
Monitoring health trackers

On subscribing to this package, you will be required to report the test readings for the following trackers.

Trackers Reporting by Patient** Frequency of Monitoring and Review
Weight Twice a month Only two response per month
Cholesterol Once in 3 months Only one response per month
Blood Pressure Once a month ( if controlled) Only one response per month
Blood Sugar Fasting and post-meal blood sugars to be reported at least once a week Only two response per month
Food Once a month or if any change responsible for low or high sugars Only one response per month
** General Instructions to Patients
  1. In case of acute or emergency conditions, DO NOT rely on the Tracker readings or feedback. Please contact your nearest hospital immediately.

Intensive Diabetes Monitoring
The intensive diabetes program is meant for Type 1/Type 2 diabetes patients who require intensive monitoring. This is ideal for patients who are diabetic and require high levels of monitoring due to infections, Coronary Artery Bypass Graft Surgery (CABG) etc

On subscribing to this package, you will be required to report the test readings for the following trackers.

Cholesterol To be reported once in 3 months Response will be sent with in 24 hrs of receiving the reading
Blood Pressure Blood Pressure readings are to be reported once a month if it is well controlled. Else it is to be reported more frequently. Response will be sent with in 24 hrs of receiving the reading
Food Report if there are any changes responsible for low or high blood sugar values. Response will be sent with in 24 hrs of receiving the reading
Blood Sugar Fasting Blood Sugar and Post-Breakfast, Post-Lunch, Post-Dinner blood sugars are to be reported daily or at minimum twice a week Response will be sent with in 24 hrs of receiving the reading
Weight To be reported once a month Response will be sent with in 24 hrs of receiving the reading
HbA1c To be reported once in 3 months Response will be sent with in 24 hrs of receiving the reading
** General Instructions to Patients
  1. For optimal dose titrations, the patient should enter at least 2 blood sugar profiles (fasting, post breakfast, post lunch and post dinner) weekly.
  2. For a patient undergoing the insulin pump treatment, additional values expected to be recorded will include pre meal and 3 AM values ( i.e. fasting, pre lunch, pre dinner, post breakfast, post lunch, post dinner and 3 AM values).
  3. Patients are requested to enter the carbohydrate content of each meal to help adjust insulin doses better. The more the number of entries, the better it will be for the monitoring doctors to analyze the data.
  4. Calibrate or troubleshoot your self-monitoring device before conducting any medical tests.

Routine Diabetes Monitoring
This is an ideal programme for Type1/Type 2 Diabetes patients who are using Insulin or Oral Anti Diabetic medicines (OAD) or both. Through our easy-to-use system, patients can report their blood sugar, insulin dosage, diet, weight, lipid profile and blood pressure. These readings will be monitored and feedback/instructions will be provided remotely.

This programme is meant for patients having hypothyroidism. With this programme, you can report the results of your thyroid function tests (T3, Free T4 and TSH) every 3 months.
